The Home at Last Service has been in operation since June 2012 as an Innovations Action Project (IAP) as part of the Victorian Government’s Homelessness Action Plan. Over the two years of the project Home at Last has become widely integrated into the housing, homelessness, health and aged care service systems across Victoria. This report outlines recommendations to extend the service as part of an older persons homelessness prevention strategy for Victoria.
