Care Finder Worker - Mornington
We are seeking a team player to join our housing support and care finder team, to provide support and assistance to older people who may be “falling through the gaps” in the system. Many older people with special needs (as defined in the Aged Care Act) will access care finders if they are vulnerable, marginalised, or disadvantaged, this includes language or cultural differences, being homeless or at risk of homelessness, and needing specialist support because of life history or identity, such as the Forgotten Australian or people who identify as LGBTIQ+.
You will be assisting older people to access and understand the aged care system and link them with services that they may require, including housing. Knowledge of the housing and homelessness system, aged care system and health and community services is desirable. As this role is focused on the Mornington Peninsula, knowledge of local services and networks will be an advantage.
You will have high levels of empathy for older people in stressful situations and a professional manner with service providers. Experience in working with older people from diverse backgrounds will be an advantage in this role, particularly experience with using interpreters or speaking a community language.
You will be using HAAG’s established community engagement strategies to proactively identify and engage with people, and build and maintain networks in the local community to support engagement with potential clients.
