We are a not-for-profit organisation specialising in the housing needs of older people. Our Home at Last program provides information and referral, housing and tenancy support and assistance to access My Aged Care and other services. The services are funded by Commonwealth and Victorian State Government grants as well as Philanthropic trusts and donations. Please note, HAAG does not own or manage any housing.

Information & Referral

A state-wide information and referral service for people in Victoria aged 50 and over, or 45 and over if Aboriginal or Torres Strait Islander, who are experiencing housing stress or homelessness, and/or are needing assistance to navigate and access aged care supports. This includes undertaking initial needs assessments and making appropriate referrals into housing and support services.

Housing Support Outreach

The Metro and Regional (Northern Victoria) Housing Support Outreach programs provide housing assistance to people aged 50 and over, or 45 and over if Aboriginal or Torres Strait Islander, who are experiencing homelessness or housing stress and who meet the priority eligibility criteria for Public and Community Housing. Outreach workers provide case management support, including assistance with the process of lodging a Victorian Housing Register (VHR) application to access public and community housing, advocacy to secure housing during the offer and sign-up process, and linking clients into services to continue living independently in the community. This service currently provides assistance to people in the northern and western metropolitan regions and the Goulburn Valley/Hume region of northern Victoria.

Care Finder

The Care Finder team provides specialist and intensive assistance to access aged care services and other supports in the community for people aged 65 years or over, or people aged 50 years or over if the person identifies as Aboriginal or Torres Strait Islander or they are homeless or at risk of homelessness. To be eligible, care finder clients must have a functional need for help with one or more activities of daily living. The program specifically targets people who are isolated, or do not have a trusted person who can assist them to access aged care service. This program currently assists clients within the North-West Melbourne Primary Health Network region, the Barwon South-West area of the Western Victoria PHN , and the Mornington Peninsula and Frankston LGAs in the South-Eastern Melbourne PHN.

Tenancy and Retirement Housing Assistance

Provides information, advice and advocacy to people aged 50 and over in Victoria who are on a low income and are residing in private rental or retirement housing including assistance with:
• Tenancy matters for people in private rentals and rooming houses, including (but not limited to) maintenance, rent increases and arrears, breach notices, and notices to vacate
• Information and advice for workers assisting people living in retirement housing in Victoria
• Contractual, financial, or residential issues relating to a Retirement Village, Caravan or Residential Park, or Independent Living Unit
• Information on Retirement Housing and Independent Living Unit options for older people on low income and have income and/or assets over the VHR priority access limit
